How to Submit Claims
  1. Filing claims on-line (Go to online claim submission) and once you submit them they are sent to the Customer Service Department.
  2. E-mail claims directly to Customer Service at CustServ@ILMGroup.com.
  3. Faxing claims directly to Customer Service at 317-704-3860.
  4. Call our Customer Service Department at 1-800-732-0777. Customer Service will take the information from you over the phone and put on Accord loss notice.

Casualty Losses
(Auto, Premises or Products Liability)

  1. Promptly report all losses to Indiana Lumbermens by calling, 1-800-732-0777. Report, any loss, even if it appears to be minor, or for which you believe you are not responsible.
  2. All losses should be reported within 24 hours of the event. Losses may be reported by fax at 1-317-704-3860.
  3. If a serious loss occurs when our office is closed, call 1-800-732-0777.
  4. An ILM Claims Representative will contact you the same day a loss has been reported if the loss is received by 12:00 noon EST.

Property Losses
(Fire, Lightning, Wind or Hail)

  1. Promptly report all losses to Indiana Lumbermens by calling, 1-800-732-0777.
  2. If a serious loss occurs when our office is closed, call 1-800-732-0777.
  3. Losses may be reported by fax at 1-317-704-3860.
NOTE:
You must take all reasonable steps to protect undamaged property from further damage. Separate damaged property from undamaged property. Maintain a record of all expense related to the loss.


Workers' Compensation Injuries
  1. Any fatality, amputation or injury requiring hospitalization should be reported to Indiana Lumbermens on the day of the injury by calling 1-800-732-0777.
  2. Complete the Employer's First Report of Injury on our website or mail/fax it to us within 24 hours. Report any injury, even if it is minor and requires no medical treatment.
  3. Losses may be reported by fax at 1-317-704-3860.
  4. Contact us immediately if the employee's condition worsens or the employee begins losing time from work.
  5. If an injured employee has lost time from work, contact us when they return to work.
  6. If Lost Time benefits are owed or a serious loss occurs, an ILM Claims Representative will contact your employee within 24 hours from the time we receive the loss notice.
